Admissions

Preadmissions Process

Our preadmissions process has been designed to maximize the likelihood that the intensive diagnostic evaluation can be completed within the desired 72-hour timeframe.

  1. Schedule a Date for Admission – Call (310) 390-2340. An admissions specialist will gather the required information including your travel plans for coming to Promises.
  2. Complete Preadmission Questionnaire – Once an admissions date has been established, clients will receive (via fax or email) a Preadmission Questionnaire and Promises Consent form. Clients must return the completed questionnaire along with signed consents for the references included in the questionnaire’s answers. These documents should be emailed to: ptpadmitdocs@promises.com or faxed to: 310-943-0485.
  3. Schedule Preadmission Interview – Schedule the interview with Dr. Skipper by calling him directly at (310) 633-4595. The interview can last up to 1-2 hours.
  4. Drug Screening – In order to receive results of drug tests prior to completion of the evaluation, it is best if samples are collected prior to coming to Promises. Clients already participating in a monitoring program may have this requirement waived with the permission of their monitoring agency.A “third party administrator” (TPA) is utilized to provide test kits and find a suitable collection site near the client’s home. A contact number will be provided and should be utilized to contact the TPA as soon as possible. Dr. Skipper will determine what combination of urine, hair or nail samples needs to be provided for testing. The required testing kits will be sent overnight to clients and the addresses and telephone numbers for testing sites near the client’s home or office will be provided. Clients should not attempt to gather samples before arriving at the approved testing site. All samples must be taken by a technician at the testing site.

Test results may take up to seven days to obtain and will be sent directly to Promises. Clients must pay testing fees through the third party administrator in advance.

  5. Client Records – Clients must gather and submit medical records and treatment records from all treatment providers including therapists, psychiatrists and treatment centers for any prior chemical dependency or other behavioral health issues. Clients must also submit board orders, complaints and /or letters from other agencies. Clients or providers should send records directly to Dr. Skipper. Records can be scanned and emailed to: ptpadmitdocs@promises.com or faxed to: 310- 943-0485. Records may also be sent to Promises:

    Chris Weber
    Promises Treatment Centers
    2045 S. Barrington Ave – Suite #B
    Los Angeles, CA 90025
    (310) 268-7737

We recommend overnight delivery to insure timely receipt of the records. Though no signature will be required, a Promises staff member is available 24 hours a day, seven days a week (including holidays) to accept deliveries and protect the confidentiality of this information. Completing the preadmission process allows the treatment team to efficiently complete an intensive diagnostic evaluation and/or develop a treatment plan allowing clients to maximize their investment of time at Promises.
